Connecting with Innovation Counseling Services

Once you are connected with a VR counselor you can inform them of your interest in psychotherapy services with Innovation Counseling Services.

Once a discussion about informed choice is concluded, VR counselors should complete a referral for psychological services (Rs-4d) and place psychotherapy CPT code 90837 on your consumer's Individualized Plan of Employment.

Ready to make a referral? Issue an authorization for 10 sessions to Innovation Counseling Services Tax ID# 83-3947446. Send Completed referral form, IPE, and authorization, along with any pertinent information to Referrals@innocounseling.com
